1 The designated premises supervisor shall have at least 2 year’s experience of managing similar premises with a similar capacity and with a similar terminal hour
2 A personal licence holder shall be on the premises at all times during which intoxicating liquor is being sold or supplied to customers or consumed by customers, except in the case of an emergency
3 Whenever the designated premises supervisor is not at the premises another personal licence holder will be nominated by the designated premises supervisor as being the responsible person to manage the premises and will have the contact details of the designated premises supervisor. The details of such person to be on prominent display outside the premises
4 A door supervisor registered with the SIA will be on duty from 20.30 hours on Friday and Saturday nights and at 22.30 hours on midweek nights. This door supervisor will be for the first 100 customers and an additional door supervisor will be on duty for every additional 100 customers or part thereof - up to a maximum of fire door supervisors
5 Door supervisors registered with the SIA will remain outside the premises until the customer have cleared the area outside the premises
6 Door supervisors registered with SIA shall be equipped with an appropriate method of keeping an accurate record of the capacity levels, including any separate capacity levels for individual rooms or levels
7 Door Supervisors shall be positioned at each entrance/exit (other than fire only exits) to ensure that no customer leaves with any bottles or glasses
8 Door supervisors will patrol the queue to the premises as necessary but no less than once every 5 minutes to ensure that customers who are drunk or who have acted in a disorderly manner, or have behaved in such a way as to cause a public nuisance or disturbance, are prevented from entering the premises
9 The designated premises supervisor/premises licence holder will maintain a daily record comprising of the following: - Start time and finish time of each door supervisor. The door supervisor will record their SIA badge number and will sign and print their name in a legible form at the beginning and end of each tour of duty. The record shall be kept securely on the premises for at least 6 months and shall be produced on request to the police or other authorised person
10 A crime prevention survey is to be carried out by the police and their recommendations fully complied with by the premises licence holder prior to the premises opening/the variation taking effect
11 To install, maintain and operate a CCTV system, which complies with the minimum requirements, expected of the Lancashire Constabulary and shall record during all hours that a licensable activity takes place on the premises
12 A dispersal policy, which is agreed by the police to be implemented at all times
13 Any person who looks or appears to be under the age of 21 shall be asked to provide identification that they are over the age of 18. The following are the only forms of identification acceptable: UK photo driving licence; Passport; PASS card (proof of age standards scheme)
14 The maximum number of persons (including staff) allowed to be on premises at any time shall be 550 (350 on the ground floor and 200 on the first floor)
15 No entry to or re-entry to the premises one hour (Friday, Saturday and Sunday 30 minutes midweek) prior to the terminal hour by members of the public or by any guests or friends of members of staff or the premises licence holder
16 The premises licence holder and/or the designated premises supervisor or a person nominated by them shall be a member of and regularly attend meetings of any pubwatch scheme for the area within which the premises are located and would include registration and use of community/network radio
17 The premises licence holder will provide to the police a drugs policy for the premises which shall as a minimum contain all of the best practices outlined in the guidance published by the Home Office and the London Drug Policy Forum ‘Safer Clubbing’
18 Free drinking water to be made available at all times from the bar
19 The provision of off-sales to terminate at 23:00 hours
20 There will be no adult entertainment services, activities, other entertainment or matters ancillary to the use of the premises that may give rise to concern in respect of children
21 No nuisance shall be caused by noise coming from the premises or by vibration transmitted through the structure of the premises
22 All external doors and windows shall be kept closed when regulated entertainment is being provided except in the event of an emergency
23 The premises licence holder or his representative shall conduct regular assessments of the noise coming from the premises on every occasion the premises are used for regulated entertainment and shall take steps to reduce the level of noise where it is likely to cause a disturbance to local residents. A written record shall be made of those assessments in a log book kept for that purpose and shall include, the time and date of the checks, the person making them and the results including any remedial action
24 There shall be placed at all exits from the premises in a place where they can be seen and easily read by the public, (or member and their guests) notices requiring customers to leave the premises and the area quietly
25 Refuse such as bottles shall be disposed of from the premises at a time when it is not likely to cause a disturbance to residents in the vicinity of the premises
26 Door supervisors and other members of staff to request customers to leave quietly and have respect for their residential neighbours
Before entertainment is provided the area of the premises designated for that purpose shall be fitted with a suitable automatic noise limiting device approved by the local licensing authority. The location, installation and settings of the noise-limiting device shall be agreed by the local licensing authority prior to any entertainment being provided. All entertainment events shall be conducted in the designated area and shall draw all electrical power from sockets served by the noise-limiting device. The noise-limiting device shall be operating correctly and in the manner intended during all entertainment events. The settings of the noise-limiting device shall be secure and tamper-proof and shall not be altered without the prior written approval of the local licensing authority
