Offered Conditions

1. A personal licence holder shall normally be on the premises at all times after 20.00 hours during which intoxicating liquor is being sold or supplied to customers or consumed by customers except in the case of an emergency
2. A minimum number of 1 member of door staff registered with the SIA will be on duty from 23.00 hours Sunday to Thursday when the premises are to be open for the sale of alcohol after 01.00 hours the following day; a minimum number of 1 member of doorstaff registered with the SIA shall be on duty from 22.00 hours on Friday; a minimum number of 2 members of door staff registered with the SIA will be on duty from 22.00 hours on Saturday with one starting at 21.00 hours.
3. Door supervisors registered with the SIA shall be equipped with an appropriate method of keeping an accurate record of the capacity levels including separate capacity levels for individual rooms or levels
4. Door supervisors shall be positioned at the entrance/exit (other than fire only exits) to ensure that no customer leaves with any bottles or glasses or food any container or wrapper
5. The DPS will maintain a daily record comprising of the following: start time and finish time of each door supervisor; the door supervisor will record their SIA badge number and will sign and print their name in a legible form at the beginning and end of each tour of duty; the record shall be kept securely on the premises for at least 6 months and shall be produced on request to the police or other authorised person
6. A crime prevention survey is carried out by the police and their recommendations fully complied with by the premises licence holder prior to the premises opening/the variation taking effect
7. Only glasses which are made of “toughened glass” shall be used whether alcohol is being consumed on the premises or in any area outside the premises which trades as part of the premises
8. To install maintain and operate a CCTV system which complies with the minimum requirements expected by the Lancashire Constabulary and shall record during all hours that a licensable activity takes place on the premises
9. Any person who looks or appears to be under the age of 18 shall be asked to provide identification that they are over the age of 18. The following are the only forms of identification acceptable: - UK photo driving licence Passport PASS card (proof of age standards scheme)
10. No entry to or re-entry shall be permitted to the premises one hour prior to the terminal hour by members of the public or by any guests or friends of members of staff or the premises licence holder
11. The DPS or a person nominated by them shall be a member of and regularly attend meetings of any Pubwatch scheme for the area within which the premises are located and would include registration and use of community/network radio
12. Where a door(s) on an escape route opens against the direction of travel, it shall be locked in the open position by a tamper-proof fastening when ever there are public on the premises
13. Other than 2 Gaming Machines and 1 Cigarette Machine there will be no adult entertainment services, activities, other entertainment or matters ancillary to the use of the premises that may give rise to concern in respect of children
Live entertainment limited to two performers or karaoke
